General Conditions of Enrollment Please read this information carefully. 

 

Registration must be submitted one week prior to start date. Cancellations must be made at least one week prior to selected start date. Late cancellations will incur a $100 non-refundable fee. Camps are subject to cancellation based upon enrollment. Drop-off time: 9:00 AM, pick-up time: 3:00. We allow up to 5 minutes grace period for pick-up times, after 5 minutes we will charge a $5/minute late charge. Please call us ahead of time to let us know if you are arriving late. After-care is from 3:00 PM – 4:30 PM for $150 (sibling rate $25 savings), please arrive on time we close the museum at 4:30 PM and will charge an additional $5/minute late charge for aftercare late arrivals past 4:35 PM. 

 

This information is correct to the best of my knowledge, and the person herein described has permission to engage in all prescribed camp activities except as noted. I hereby give permission to Southern California Children’s Museum to provide routine health care, administer prescribed medications, and seek emergency medical treatment including ordering x-rays or routine tests in the event of an emergency when I cannot be reached. 

 

I agree to the release of any records necessary for insurance purposes. I give permission to the camp to arrange necessary related transportation for my child. In the event I cannot be reached in an emergency, I hereby give permission to the physician selected by the Museum to secure and administer treatment, including hospitalization, for the person named above. 

 

This completed form may be reproduced for off-site trips. I allow images of my child to be used in any promotional material. I have read and fully agree with all the terms of registration as stated on this page and all prior pages. 

 

Liability Waiver and Release 

Southern California Children’s Museum provides an exciting and recreational indoor and outdoor environmental arts-based program for children. Outdoor play consists of running, jumping, walking, water play, and more all of which involve certain risks of injury. I am familiar with the risks associated with participation in these activities, and I hereby waive and release Southern California Children’s Museum. from any and all claims for injuries related to my child’s participation in these outdoor activities which may include mistakes made by Southern California Children’s Museum employees or other campers. Southern California Children’s Museum cannot be held responsible for information not shared with us regarding your child’s health and/or limitations. 

 

All vendors and party designs are subject to review by the SCCM team and vendors must name SCCM as an additional insured. While additional set-up and take-down time are possible, SCCM reserves the right to deny additional time based on our schedule, staffing, and availability at the time of booking and up until the day of the event. Should this be necessary we will refund your purchase of additional time. All adjustments to time, guest count, and vendor details must be confirmed in writing by SCCM prior to booking vendors and sending out party invitations. Upon approval of additional party guests, SCCM will charge the normal entry fee of $10/person (children under 1 free, military/senior/Museum for All discounts applied where applicable) on the day of the event.  The final guest list is required at least 1 week prior to the event. Where possible SCCM welcomes party guests to enjoy all-day access to the museum, however, we reserve the right to limit access based on our programming and buy-out partners up until the day of the event. Verbal confirmations including conversations in-person or via phone are not considered final terms for your event. All details, adjustments, and additional details beyond the reservation terms listed through our online scheduling system must be confirmed via email with SCCM staff. 

 

The Southern California Children’s Museum continues to monitor policy and procedures during the COVID-19 pandemic. The safety and well-being of our community, including guests and visitors, is our top priority. We are collaborating with federal, state, and local agencies to implement best practices aimed at managing the spread of the novel and complex virus.

SCCM has put in place preventative measures to reduce the spread of the COVID-19 virus; however, under no circumstances, is SCCM able to guarantee that you or a guest will not become infected with the virus while on our premises and in surrounding areas.

Adherence to SCCM’s event policy and guidelines will be continuously monitored and enforced by museum personnel. Please review the agreement below. Additionally, your signature will be required as a confirmation of your intent and commitment to abide by policies put in place to protect museum visitors and staff.
